You will be required to login, or you can register and create a login ID if you do not have one.
Applications can be saved and updated as needed.

(These can be uploaded to the application when applying)

✔	Copy of Child's Birth Certificate
✔	Proof of Residency (Property deed, rental agreement, property tax statement, building permit, voter registration card, residency affidavit)
✔	Child's Immunization Record
✔	Custody Documents (if applicable)
✔	Copy of ETR/IEP (if applicable)

New and existing students will need to complete an Emergency Medical Form
