✔	Proof of Residency (deed, rental agreement, tax statement, utility bill, building permit, voter registration card)
✔	Child's Immunization Record
✔	Custody Papers (if applicable)
✔	Proof of Income (two most recent pay-stubs, previous year’s income tax return, or statement from caseworker/SSI)
	Note:No Income Documentation is required under the following terms:
	•	Children on an IEP
	•	Children on a case/service plan with Children's Services (documentation required)
	•	Children placed in kinship care as documented through Kinship Permanency Incentive Program Payments (must provide proof)
	•	Family receives publicly funded childcare with $0 co-pay (must provide proof
