✔	Proof of Residency (Property deed, rental agreement, property tax statement, building permit, residency affidavit)
✔	Child's Immunization Record
✔	Custody Documents (if applicable)
✔	Copy of ETR/IEP (if applicable)

New and existing students will need to complete an Emergency Medical Form
